Out of curiosity I decided to play around with this. It seems to line up by missing the first row of labels, and then also printing below the last row. I did print a plain paper version, and compare it to the labels. (Although chances are it will turn out to be an OOo bug, which the Neo project can't fix). That would let me do some more testing and decide if we need to file this as a bug.

Would you be willing to measure the actual labels and provide correct numbers for all the dimensions listed in the New>Labels>Options pane, plus the actual paper size (i.e. My best guess is that it is an bug and not a Neo specific problem, but I don't have to test it against. I've had this kind of problem with labels before. If you don't want to waste labels, you could do this on plain paper and compare against the labels. (Since the page of labels is actually slightly longer than Neo thinks it is). With this as the cae, you might try printing a page of labels and see if it lines up correctly despite the discrepancies. Correcting the page size there only messes up the labels. Specifically, the width is 8.55 and the height is 10.52 instead of 8.5 x 11. The other thing I noticed just noticed is that if you have the label document up and choose Format>Page and click on the Page tab, the page dimensions are (presumably) incorrect. Too bad this just doesn't work correctly! I'll have to try to play with the margins. Trouble is, the actual labels have an equal margin at the top and bottom of the page. Physically measure the margins, pitch, etc., and make sure the numbers are correct. That, at the ruler markings do not seem to add up to 11, even accounting for the top margin. 5 inches, but that the last row of the labels is at the very bottom of the page there is no bottom margin. When I went back to the document, I noticed that the top margin is. (I used the Preview button in the standard Mac print dialog, not the preview button in the toolbar). I don't have the actual labels to compare. I tried it myself, and the top margin is larger than the bottom margin.

I cannot figure out how to change this to be in the right place. The whole thing should be centered on the page. The top margin is down about 1/2 inch from where it should be, and the bottom label row is right at the bottom. But when I preview the labels, they are not formatted in the middle of the page. I then format the first one, and click synchronize. I pick letter size avery, 5167, which is the label I have, and I check the synchronize check box. So, I pick new>labels, and up comes the labels dialog box. OK, so I am trying to print a page of return address labels.
